Form filling instructions

For all awards except TechnoShield, nominations are accepted. (download nomination pdf or word file in each category)

For all awards, the applicant may directly apply for the award (download application pdf or word file in each category)

Download both PDF and word document versions of the application forms online.

Hand fill or print the relevant details in the PDF Version of the application form.

Paste photograph and get authorized signature on the hard copy version of the application form.

The word document version of the application form has to be filled in a soft copy version only.

All annexure and attachments to be sent in the soft copy version on a CD only, with a soft copy of the application form.

All fields in the application forms have to be duly filled with relevant information

Nomination forms can only be filled by the nominating entity with the consent of the nominee only.

Only hard copies of the application forms have to be sent with the CD, containing all annexure and attachments.
